The absence of urease enzyme in mammalian cells indicates its existence in the stomach.Â
Urea gets split at the interface between lumen and gastric epithelium by H. pylori’s gastric urease, which yields CO2 and NH3.Â
Then as you breathe out, the blood picks up 14CO2 and releases it.Â

Once the patient has consumed the 14C-urea capsule, the sample obtained for testing is intended solely for in vitro diagnostic applications.
Patients with achlorhydria may experience a false positive test result. Rarely can urease linked to Helicobacter species other than H. pylori (e.g., Helicobacter heilmanni) cause a false positive test.
