The United States Medical Licensing Examination (USMLE) program is rolling out updated test delivery software to improve the experience for examinees. Step 3 exams have already transitioned to the new system as of March 10, 2026. Step 2 Clinical Knowledge (CK) exams administered on or after May 7, 2026, and Step 1 exams administered on or after May 14, 2026, will also be delivered using the updated software.
The updated test delivery software introduces several improvements to the Step exam interface. Examinees can observe a new design and better navigation with the keyboard, and the interface will be more intuitive. Other new features include a settings menu and the ability to customize the contrast of individual images, making them more convenient and easier for examinees to adjust during the exam.
Although the exam content, total number of items, and overall exam duration remain unchanged, the structure of the exam blocks has been revised. The new format increases the number of blocks while shortening their duration. As before, examinees can only review questions within the current block and cannot return to previous blocks once closed.
For Step 2 CK, launching on May 7, 2026, the total number of multiple-choice questions remains 316. The new format doubles the number of blocks from 8 to 16, reduces the number of items per block from 38-40 to 18-20, and shortens block time from 60 minutes to 30 minutes. Despite these changes, the total exam day length remains 9 hours.
Similarly, Step 1, launching on May 14, 2026, retains its total of 280 multiple-choice questions. The number of blocks will increase from 7 to 14, with each block containing 20 items instead of 40, and the block duration will be reduced from 60 to 30 minutes. The overall exam day length continues to be 8 hours.
The USMLE program has offered testing experience, tools, and tutorials on Step 1, Step 2 CK, and Step 3. Examinees are advised to review the resources beforehand to familiarize themselves with the new interface and the 30-minute block format. The software update is also informed by examinee feedback and reflects the USMLE program’s commitment to delivering Step exams in a proficient, reliable, and efficient manner.
It is aimed at offering a testing platform that can keep up with the changing demands of the examinees without compromising the integrity of the tests. Also, the new software will synchronize the Step exam experience with other USMLE co-sponsor NBME exams, since self-assessment and practice materials will already be provided on the same platform. Such consistency is aimed at making the examinees feel more familiar and viable as they move up the USMLE sequence.
The USMLE program aims to improve usability and ease of navigation, providing candidates with the best tools to perform effectively while maintaining the same exam content and duration, and modernizing both the interface and exam structure.
