A 65-year-old female presented with complaints of shortness of breath on exertion and a non-productive cough. She mentioned that she used to smoke in the past and has had a history of hypertension for the past ten years. The patient mentioned that she had visited a healthcare professional eight months ago with similar complaints and was prescribed bronchodilators, oral steroids, and empiric antibiotics. However, she experienced minimal improvement. On physical examination, diffuse rales and mild wheezing were detected in the chest region. The patient's oxygen saturation was 90%, and her blood pressure was measured at 130/90 mmHg. A chest X-ray revealed reduced lung volume and coarse reticulation. Further examination using a CT scan showed ground glass opacities and consolidation. To investigate further and rule out malignancy, a lung biopsy was performed, which revealed diffuse alveolar damage.
