Actions and Spectrum:
fosfomycin is a broad-spectrum antibiotic that acts by inhibiting bacterial cell wall synthesis. It is active against a wide range of gram-positive and gram-negative bacteria.

Black Box Warning:
It is essential to follow the dosage and administration instructions for the medication carefully.
Contraindication/Caution:
Hypersensitivity: individuals with a known hypersensitivity to fosfomycin or other components of the formulation should not use it.
Renal impairment: patients with severe renal impairment may require dose adjustments or close monitoring of their kidney function.
Resistance: fosfomycin resistance is becoming more common, so it is important to consider alternative antibiotics if the patient has a history of treatment failure with fosfomycin.
fosfomycin is not indicated for the treatment of pyelonephritis or perinephric abscess.
Administration: intravenous administration of fosfomycin can cause local adverse reactions and should be administered slowly over a period of several minutes.
Pregnancy consideration:
Category B.
Breastfeeding warnings:
The drug is excreted in breast milk. Hence not recommended during breastfeeding.
Pregnancy category:
Category A: well-controlled and Satisfactory studies do not show risk to the fetus in the first/later trimester.
Category B: there was no evidence of risk to the fetus in animal studies, and there were not enough studies on pregnant women.
Category C: there was evidence of risk of adverse effects in animal reproduction studies, and no adequate evidence in human studies must take care of potential risks in pregnant women.
Category D: adequate data available with sufficient evidence of human fetal risk from various platforms, but despite the potential risk, and used only in emergency cases for potential benefits.
Category X: Drugs listed in this category outweigh the risks over benefits. Hence these categories of drugs need to be avoided by pregnant women.
Category N: No data is available for the drug under this category.
Pharmacology
Pharmacodynamics:
fosfomycin is a broad-spectrum antibiotic that works by inhibiting bacterial cell wall synthesis. It acts by inhibiting the enzyme MurA, which is responsible for the formation of the peptidoglycan layer of the cell wall. This results in bacterial cell death.
Pharmacokinetics:
Pharmacokinetics studies how a drug is absorbed, distributed, metabolized, and eliminated by the body. The following parameters can describe the pharmacokinetics of fosfomycin.
Absorption
The drug is rapidly absorbed
Fasting bioavailability is 37%, whereas, with food, it is 30%.
Distribution
The volume of distribution is 90-180L.
Metabolism
fosfomycin is metabolized in the body.
Elimination and excretion
The half-life is 3-8 hours
The drug is excreted in through urine.
Administration:
Dissolve a pack of 3 gm powder in 3-4 oz water and take it orally.
