kanamycin

Adult and Children
IM:

7.5

mg/kg

every 12 hrs

per day (do not exceed 1.5g/day)
IV: 5 to 7.5 mg/kg/dose every 8 to 12 hours per day (do not exceed 1.5g/day)



Dose Adjustments

For renal impairment:
CrCl 50-80 ml/min: Reduce the dose 10 to 30%
CrCl 10-50 ml/min: Reduce the dose 40-30%
CrCl <10 ml/min: reduce the dose 40-60%

fusidic acid 

Indicated for Susceptible staphylococcal infections such as osteomyelitis, septicemia, endocarditis, superinfected cystic fibrosis, cutaneous infections, pneumonia, and wound infections
:


Tablets-Administer 250mg twice a day or 500 to 1000mg thrice a day Suspension-750 to 1000mg thrice a day



cefapirin 

Administer dose of 0.5 to 2 g daily intramuscularly or intravenously in 1 or 2 daily doses



latamoxef 

2 g daily Intramuscular injections in two divided doses



latamoxef 

2 to 6 g daily Intravenous injections in 2 to 3 divided doses



Dose Adjustments

Renal Impairment
Haemodialysis: 15 mg/kg intravenously at the end of each dialysis
CrCl above 30 mL/min: 15 mg/kg every 1-2 times a day
CrCl below 10 mL/min: 15 mg/kg every 36 to 48 hours
CrCl 10 to 30 mL/min: 15 mg/kg every 24 to 36 hours

cefradine 

1-2 gm orally each day in 2-4 divided doses
2-4 intravenously each day over 3-5 minutes
Keep the maximum dose to 8 gm each day
Dose Adjustment
In the case of renal impairment,
When CrCl > 20 ml/min, 500 mg every 6 hours
When CrCl < 5 ml/min, 250 mg every 12 hours
When CrCl is 5-20 ml/min, 250 mg every 6 hours



sisomicin 

Gram-negative & staphylococcal bacteria :

3 mg/kg daily in 2 to 3 divided dosages. If required, it may give as Intravenous infusion



cefpiramide 

1 to 2 g is given as cefpiramide sodium through Intravenous infusion in two divided doses



chloramphenicol 

Infections Caused by Susceptible Strains
:


Administer 50 mg/kg/day divided every 6 hours intravenously; in extreme circumstances, patients with severe infections or organisms with a moderate level of resistance may need to take up to 100 mg/kg/day.



phthalylsulfathiazole 

Indicated for Susceptible infections, bowel cleaning before surgery:


Day 1: Administer 1g four hours every day. Do not exceed 6g in a day.
Days 3 to 4: Administer 1g six hours every day. Do not exceed 4g in a day.
Days 5 to 7: Administer 1g eight hours every day. Do not exceed 3g in a day.



gentamicin 

Dosing Conventional
3-5 mg/kg daily intravenous/intramuscular divided every 3 times a day
Extended dosing interval
Initially: 5 to 7 mg/kg/dose intravenous daily
gentamicin levels should be measured between 6 and 14 hours after starting the gentamicin infusion



 

fusidic acid 

Indicated for Susceptible staphylococcal infections such as osteomyelitis, septicemia, endocarditis, superinfected cystic fibrosis, cutaneous infections, pneumonia, wound infections

<1 year old:
Suspension-Administer 50 mg/kg/day divided thrice a day
1 to 5 years old:
Suspension-Administer 250mg thrice a day
>5 to 12 years old:
Tablets-Administer 250 to 500 mg thrice a day
Suspension-500 to 1000mg thrice a day



spiramycin 

Based on the body weight-(150,000 units/kg) stated as 750,000 units of Rovamycine 250 orally every day
The daily intake is divided into 2-3 doses
For 30 kg of body weight: Take six capsules every day
For 20 kg of body weight: Take four capsules every day
For 15 kg of body weight: Take Three capsules every day
Note: For severe infections, the recommended dosage can be raised by 50%



latamoxef 

50 to 100 mg/kg/day Intramuscular injections in 2 to 3 divided doses



latamoxef 

50 to 100 mg/kg/day Intravenous injections in 2 to 3 divided doses



netilmicin 

Premature infants & neonates less than 1 week: 6 mg/kg/day in divided dosages every 2 times a day
Infants and neonates more than1 week: 7.5 to 9 mg/kg/day in divided dosages every 3 times a day
older children: 6 to 7.5 mg/kg/day in divided dosages every 3 times a day

Renal impairment
CrCl 20-40 mL/min: 4 to 6.5 mg/kg administered once a day or in divided dosages everyday
CrCl 40-60 mL/min: 4 to 6 mg/kg administered once a day or in divided dosages every 2 times a day
Liver impairment
dose adjustment is not required

cefradine 

50-100 mg/kg intravenously each day in 4 divided doses
Increase the dose to 200-300 mg/kg each day for severe infections

In the case of renal impairment,
When CrCl > 20 ml/min, 500 mg every 6 hours
When CrCl < 5 ml/min, 250 mg every 12 hours
When CrCl is 5-20 ml/min, 250 mg every 6 hours
25-50 mg/kg each day in 2-4 divided doses
Keep the dose in the range of 75-100 mg/kg each day divided into 2-4 doses for otitis media
Keep the maximum dose to 4 gm each day



ceftizoxime 

In children who are below six months or equal, 50 mg/kg is given via IV three or four times a day



chloramphenicol 

Children and infants: As in adults, administer up to 100 mg/kg/day to achieve appropriate cerebrospinal fluid concentrations; however, the dose should be lowered as soon as feasible to 50 mg/kg/day.
Infants and children suspected of having underdeveloped metabolic functions: Administer 25 mg/kg/day divided into six hours will result in blood levels of the medication that are therapeutic.
Neonates (<28 days):
Loading dose-Administer 20 mg/kg intravenously and the maintenance dosage 12 hours apart.
Maintenance dose:
Infants under 7 days old: Administer 25 mg/kg intravenously once a day Infants older than 7 days and weighing less than 2000 g: Administer 25 mg/kg intravenously once a day.
Over 2,000 g and older infants: Administer 50 mg/kg/day intravenously twice a day

Children under three years: Administer 0.2 g/kg body weight every 8 hours

Infants: 2.5 mg/kg dosage intravenous/intramuscular every 3 times a day
Below 30 weeks of gestation
0 to 28 days: 2.5 mg/kg daily intravenous/intramuscular
Above 28 days: 3 mg/kg daily intravenous/intramuscular
30 to 36 weeks of gestation
0 to 14 days: 3 mg/kg daily intravenous/intramuscular
Above 14 days: 5 mg/kg daily intravenous/intramuscular divided every 2 times a day
Above 36 weeks of gestation
0 to 7 days: 5 mg/kg daily intravenous/intramuscular divided every 2 times a day
Above 7 days: 7.5 mg/kg daily intravenous/intramuscular divided every 3 times a day
