Dose Adjustments
Renal impairment:
CrCl ≥30 mL/minute: Initial and maintenance: No dosage adjustment is necessary
CrCl <30 mL/minute:
Initial: No dosage adjustment necessary; use caution in maintenance dose
Hepatic impairment:
Mild to moderate: no dosage adjustment is necessary
Severe: Not recommended
