Actions and spectrum:Â
It belongs to a diabetic class of drugs known as thiazolidinediones. It lowers blood sugar by improving the body’s sensitivity to insulin. Troglitazone works uniquely by relying on insulin’s presence to become activated. It lowers the glucose production in the liver and increases glucose uptake in muscles when insulin is present. The troglitazone interacts with protein receptors (PPARs) in the cell nuclei which controls the genes involved in regulating glucose and fat metabolism.Â

Black Box WarningÂ

Contraindication/Caution:Â
Contraindication:Â
It is known to cause liver diseases leading to liver death.Â
AnorexiaÂ
HypersensitivityÂ
Caution:Â
It may lower the activity of birth control medication.Â
Pregnancy consideration:
US FDA pregnancy category: B
Lactation:
No sufficient study data
Pregnancy category:
Category A:Â Studies that were well-controlled and met expectations revealed no risk to the foetus `in either the first or second trimester.
Category B: There were a lack of studies on pregnant women and no evidence of risk to the foetus in animal experiments.
Category C:Â there was evidence of risk of adverse effects in animal reproduction studies, and no adequate evidence in human studies must take care of potential risks in pregnant women.
Category D:Â adequate data with sufficient evidence of human fetal risk from various platforms, but despite the potential risk, and used only in emergency cases for potential benefits.
Category X:Â Drugs listed in this category outweigh the risks over benefits. Hence these categories of drugs need to be avoided by pregnant women.
Category N:Â There is no data available for the drug under this category
PharmacologyÂ
It is a diabetes medication taken orally that improves blood sugar levels by reducing the body’s resistance to insulin.Â
PharmacodynamicsÂ
It is a diabetes medication taken orally that improves blood sugar levels by reducing the body’s resistance to insulin. It is combined with sulfonylureas or insulin to enhance blood sugar control.Â
Pharmacokinetics:Â
AbsorptionÂ
It is absorbed rapidly in the g.i.t and its absorption may be increased with by food to the extent of 30-85%Â
DistributionÂ
99% of troglitazone is bound to serum albumin.Â
MetabolismÂ
It undergoes in liver forming two important metabolites such as sulfate conjugate metabolite and quinone metabolite.Â
Elimination and excretionÂ
Majorly 85% of the drug is eliminated through faeces and 3% of the drug is eliminated in urine.Â
The elimination half-life is 16-34hours.Â
AdministrationÂ
It is administered orally in the form of tablets.Â
