0.4-0.5 mg/kg IV over 1 minute as an initial dose 
0.08-0.1 mg/kg for maintenance dose
To maintain neuromuscular block 20-45 minutes after the initial dose, repeat the maintenance dose every 15-25 minutes as needed
If intubation is preceded by succinylcholine, the continuous infusion should be 0.005-0.01 mg/kg/min IV or 0.2-0.4 mg/kg IVP
