Class :
Thyroid Products
Dosage forms& StrengthsÂ
Each 60 mg tablet equals 60-65 mg (1 grain) of the desicated thyroid.  Â
Liothyronine sodium (T3) is around four times as effective as levothyroxine (T4).  Â
T3/T4, tablet Â
37.5/150mcg: Thyrolar 3 (180mg) Â
25/100mcg: Thyrolar 2 (120mg) Â
12.5/50mcg: Thyrolar 1 (60mg) Â
6.25/25mcg: Thyrolar 1/2 (30mg) Â
3.1/12.5mcg: Thyrolar 1/4 (15mg) Â
1 tablet Thyrolar 1/2 daily, followed by 1 tablet Thyrolar 1/4 every 2 to 3 weeks
In long-standing myxedema, a lower initial dose of 1 tab is recommended, especially if cardiovascular damage is suspected, in which further caution is advised
Maintenance: one tablet of Thyrolar 1 to 1 tablet Thyrolar 2 daily; failure to respond to tab Thyrolar 3 can suggest a lack of compliance and malabsorption.
After necessary clinical laboratory analyses of T4 bound and free TSH blood levels, adjust the dosage within the first four weeks of therapy

Actions and spectrum:Â
The hormones T4 and T3, made by the thyroid, are amino acid-based and are dependent on iodine for their synthesis. The main thyroid hormone that is secreted is T4; it becomes the more active T3 with the help of deiodinases in the peripheral tissues. T3 is responsible for the increase in the basal metabolic rate, protein synthesis alteration, and the body’s sensitivity to catecholamines like adrenaline. These hormones are the main factors in the correct development and differentiation of all human cells, as well as the regulation of protein, fat, and carbohydrate metabolism. They have the largest effect on the cellular use of energy compounds. Thyroid hormone derivatives attach to thyroid hormone receptors to induce downstream effects.Â

Pharmacology:Â
It is a thyroid hormone replacement therapy that is derived synthetically. It contains thyroxine, T4 (levothyroxine sodium), and triiodothyronine, T3 (liothyronine sodium) in 4:1 ratio as per weight.Â
Pharmacodynamics:Â
Thyroid hormone drugs can be naturally occurring or man-made which can contain T4, T3, or both. T4 and T3 which are built in the human thyroid gland are made through a reaction that involves iodine and the amino acid tyrosine. Liotrix is a synthetic preparation which is a combination drug of T4 and T3 in a 4:1 weight-based ratio. These hormones increase the consumption of oxygen by most cells in the body and so the basal metabolic rate is increased which consequently results in the enhanced burning of carbohydrates, lipids, proteins. They fundamentally influenced every body system and are particularly vital for the development of the central nervous system.Â
Pharmacokinetics:Â
Elimination and excretion: the drug is known to be eliminated mainly through kidneys.Â
Administration:Â
It is recommended to take this tablet orally once daily on an empty stomach at least half an hour before breakfast.Â

Generic Name: liotrixÂ
Pronounced: (lie-oh-trix) Â
Why do we use liotrix?Â
Liotrix is a substitute therapy for hypothyroidism which is the ailment of the thyroid not being able to produce sufficient thyroid hormones. It provides both T4 and T3 hormones, rectifying the thyroid hormone levels, subsiding symptoms, and restoring normal thyroid function. It is most effective when patients are already on levothyroxine monotherapy but it doesn’t work properly (TSH usually remains elevated) and liothyronine (T3) is added to give immediate relief.Â
