Actions and Spectrum:

  • Neutralization of Gastric Acid: magnesium carbonate acts as an antacid by neutralizing excess stomach acid. When it comes into contact with hydrochloric acid in the stomach, it undergoes a chemical reaction in which magnesium ions (Mg2+) are released and combined with the acid’s hydrogen ions (H+). This results in the formation of magnesium chloride and carbon dioxide gas (CO2), thereby reducing the acidity in the stomach.
  • Relief of Heartburn and Acid Indigestion: magnesium carbonate effectively relieves heartburn and acid indigestion caused by conditions like gastroesophageal reflux disease (GERD) or overindulgence in acidic or spicy foods.
  • Upset Stomach: It can also help alleviate symptoms of an upset stomach, such as discomfort or pain, often associated with excess stomach acid.

Black box warning:

None

Contraindications/caution:

Contraindications:

  • Allergy or Hypersensitivity: Individuals with a known allergy or hypersensitivity to magnesium carbonate or its components should avoid its use.
  • Kidney Impairment: People with severe kidney impairment or kidney failure may have difficulty excreting excess magnesium, leading to elevated magnesium levels in the blood (hypermagnesemia). In such cases, magnesium-containing medications like magnesium carbonate should be used with caution or avoided altogether.
  • Bowel Obstruction: magnesium carbonate may not be suitable for individuals with bowel obstructions or conditions that prevent normal gastrointestinal motility, as it could exacerbate the obstruction.
  • Myasthenia Gravis: Myasthenia gravis is a neuromuscular disorder, and using magnesium-containing antacids may worsen muscle weakness in people with this condition.

Caution:

  • Mild or Occasional Symptoms: magnesium carbonate is suitable for short-term relief of mild or occasional heartburn, acid indigestion, and upset stomach.
  • Dosage and Timing: Taking excessive amounts of magnesium carbonate or using it for an extended period may lead to excessive magnesium intake, adversely affecting health.
  • Kidney Function: People with kidney problems may have difficulty excreting excess magnesium, leading to elevated levels in the blood (hypermagnesemia).

Pregnancy consideration: Insufficient data available

Lactation: Excretion of the drug in human breast milk is known

Pregnancy category:

Category A: well-controlled and Satisfactory studies show no risk to the fetus in the first or later trimester.

Category B: there was no evidence of risk to the fetus in animal studies, and there were not enough studies on pregnant women.

Category C: there was evidence of risk of adverse effects in animal reproduction studies, and no adequate evidence in human studies must take care of potential risks in pregnant women.

Category D: adequate data with sufficient evidence of human fetal risk from various platforms, but despite the potential risk, and used only in emergency cases for potential benefits.

Category X: Drugs listed in this category outweigh the risks over benefits. Hence these categories of drugs need to be avoided by pregnant women.

Category N: There is no data available for the drug under this category

Pharmacology:

  • Antacid Action: magnesium carbonate is an inorganic compound that acts as an antacid by neutralizing excess stomach acid (hydrochloric acid) in the gastrointestinal tract. When magnesium carbonate reacts with stomach acid, it releases magnesium ions (Mg2+), forming magnesium chloride and carbon dioxide gas (CO2). This neutralization raises the stomach’s pH level, reducing acidity and relieving heartburn, acid indigestion, and upset stomach.
  • Systemic Absorption: While magnesium carbonate primarily exerts its effect locally in the stomach, a small portion of the magnesium ions released during neutralization may be absorbed into the bloodstream.
  • Magnesium Supplementation: The systemic absorption of magnesium from magnesium carbonate can contribute to magnesium supplementation, which benefits individuals with mild magnesium deficiency. Magnesium is an essential mineral in numerous bodily biochemical processes, such as muscle function, nerve transmission, bone health, and energy metabolism.
  • Laxative Effect: In some cases, magnesium-containing antacids, including magnesium carbonate, may exert a mild laxative effect due to the osmotic action of unabsorbed magnesium in the intestines. This laxative effect can help relieve constipation.

Pharmacokinetics:

Absorption

magnesium carbonate is an oral preparation of magnesium, and its absorption primarily occurs in the small intestine. Like other forms of magnesium, magnesium carbonate is absorbed through active transport and passive diffusion processes in the distal small intestine, mainly the ileum.

Distribution

Once absorbed, magnesium is distributed throughout the body via the bloodstream. Magnesium ions are essential for various physiological processes and involve numerous biochemical reactions in cells and tissues.

Metabolism

magnesium carbonate is not metabolized in the body; it remains in its chemical form until excretion.

Elimination and Excretion

magnesium carbonate is primarily eliminated from the body through the urine. The kidneys filter magnesium from the blood and excrete it into the urine. Some small amounts of magnesium may also be excreted through the feces.

Administration:

Oral administration

Take magnesium carbonate by mouth with a full glass of water. It is usually taken after meals or as needed to relieve heartburn, acid indigestion, and upset stomach.

Patient information leaflet

Generic Name: magnesium carbonate

Why do we use magnesium carbonate?

magnesium carbonate has various uses, primarily due to its antacid properties and role as a magnesium supplement. Here are some common uses of magnesium carbonate:

  • Antacid: magnesium carbonate is used as an antacid to relieve heartburn, acid indigestion, and upset stomach symptoms. It works by neutralizing excess stomach acid, reducing acidity in the stomach, and alleviating discomfort caused by acid-related conditions.
  • Magnesium Supplementation: magnesium carbonate can serve as a dietary supplement to provide additional magnesium to the body.
  • Laxative: Unabsorbed magnesium from magnesium carbonate can sometimes exert a mild laxative effect due to its osmotic action in the intestines. This effect can help relieve constipation in some individuals.
  • Heartburn and Indigestion Relief: As an antacid, magnesium carbonate is commonly used to alleviate heartburn and indigestion symptoms caused by conditions such as gastroesophageal reflux disease (GERD), overeating, or consuming acidic or spicy foods.
  • Acid Upset Relief: magnesium carbonate can help soothe an upset stomach caused by excessive stomach acid, relieving symptoms like discomfort and nausea.
  • Magnesium Deficiency: As a magnesium supplement, magnesium carbonate addresses mild magnesium deficiencies due to inadequate dietary intake or certain medical conditions.

None
